How it actually works

The mechanism, plainly.

As the face ages it loses support before it loses skin: the fat pads that sit over the cheekbones thin and slide, bone along the jaw and around the eye sockets recedes, and the skin above drapes differently — a flatter cheek, a softer jawline, a shadow under the eye. A hyaluronic-acid filler is a smooth gel that, placed at the right depth in the right plane, replaces that lost support. Hyaluronic acid binds water and is broken down by the body over months, and it can be dissolved on purpose if it is ever placed wrongly.

The choice that matters is where it goes and how much. Filler placed on the bone at the cheek lifts the tissue above it; the same amount placed in the soft tissue below simply makes the cheek larger, which is the “overfilled” look people fear. Different gels have different firmness and are chosen for the site: firmer for bone, softer for the lip.

About Dermal fillers.

Will I look overfilled?

Not if it is placed for structure. The overfilled look comes from volume placed in soft tissue rather than support placed on bone, and from too much of it. Our injectors place conservatively, on the structure, and would rather you come back for a little more than leave with too much.

How many syringes will I need?

Fewer than you may think when it is placed structurally — a cheek or a chin often needs one; a jawline more. The injector assesses your face at the visit and confirms the number, and the total price, before anything is placed.

How long does it last?

Nine to eighteen months, depending on the gel, the site — lips move more and metabolise faster than a cheekbone — and you. Some structure persists as the gel is absorbed.

Is it reversible?

Yes. Hyaluronic-acid filler can be dissolved with an enzyme if it is ever placed wrongly or you change your mind. That is one reason we use only hyaluronic-acid gels for structural work.

Does it hurt, and is there downtime?

Pressure and a brief sting; most gels contain a local anaesthetic and numbing cream is used where it helps. Expect possible swelling or a bruise at the site for a few days — coverable with makeup. Plan lips a week or two before anything important.

Filler or Botox?

They do opposite things. Botox relaxes a muscle that folds the skin — lines that appear when you move. Filler replaces support that has been lost — flatness or hollowing at rest. Many people need both; the injector will say which. Botox & Dysport has its own page.
